The UHC hiding the breach notice page is one thing but it looks like Nebraska is suing Change Healthcare for what amounts to pure incompetence for an organization that holds HIPAA data. If the Nebraska AG is right, they got in through a support reps username and password, then had access to the entire system, being able to transit between servers without any change in user credentials or multi factor authentication.
