    End-users of EHR systems are ultimately accountable for their safe and effective use, like any tool in clinical care.

    Of all the statements and proclamations in this white paper, the one in 4a above is most puzzling. Aside from the fact that the entire paper should have been issued at least 10 years ago, EHR systems are absolutely different from and unlike any tool in medical care, since any and all tools in medical care have a common safety thread: they have been approved by the FDA. HIT systems have not been approved by the FDA.
