		<description>Wellsoft&#039;s Medication Verification &amp; Exchange feature has been successfully integrated with MedsTracker® from Design Clinicals, Inc. at CentraState Medical Center in Freehold, New Jersey.

Wellsoft’s Medication Verification &amp; Exchange feature allows for bi-directional exchange and synchronization of codified home medication and prescription information across the hospital enterprise as well as medications administered in the emergency department. Wellsoft currently supports information exchange with First Data Bank, RxNorm, and Elsevier’s Gold Standard Clinical Pharmacology and plans to support other content providers, such as NDC, MicroMedex and Cerner® Multum.

Is this the same system that the hospital uses for medication reconciliation?
