		<description>Ugh.  Let&#039;s  disregard the fact that health insurance is not like auto insurance or that a very high deductible (e.g., 10k for families) health plan that they had to self-fund would leave most American families very hard up in terms of access.  

Let me sum up the main points of this interview:

- Doctors will always produce good outcomes if given the time and proper resources yet acknowledges later the deficiencies of many doctors while touting his own reputation.

- Doctors generally produce good outcomes yet you can&#039;t systematically measure these same doctors&#039; performances because of variations in their patient panels.  Instead, it will be solely based on patient hear-say and word-of-mouth.

- Says that EBM doesn&#039;t exist and he isn&#039;t trying to enforce standards on other providers yet he is trying to come up with his own standards to treat his patients.

- EBM is psuedo-rationing yet he is setting up a practice where patients are rationed by what they get by ability to pay.

- Acknowledges the myriad of problems with coordination of care because of the current reimbursement system and fragmentation of physician practices but is setting up a practice that does not really fundamentally address either one of these problems.

Inconsistencies galore.  Basically this guy seemingly wants a return of medical profession to the supposed glory days of the 1970s when PCPs worked more of a 9-5 job and largely did as they saw fit with very minimal external accountability.

I understand some of his professional angst but if he wanted to get paid like an investment banker or a lawyer then he should have gotten an MBA or his JD instead.  BTY, he also would have to likely work more hours and have less job security if he had chosen either one of those professions instead of becoming an MD.</description>
